Beginning
with calendar quarter ending September 30, 2004, the quarterly
worker's compensation fee paid on form WC-1 increases from
$4 to $4.30 per covered worker (employee). Only the employer's
share increases. |

The set-up fee has been lowered from $100.00 to $25.00, the
$5.00 per processing charge has been lowered to $3.00, and
the $1.50 per employee fee has been reduced to $1.00. See
